Ok brothers here it is. Our attempt to boost the participation on the boards.
If you or your department goes to a job, post it up. Maybe we can learn something from the post and start a training based discussion on it.
For the format, lets go with Department Name Date Size-up Conditions Actions taken.
This will be the test run for the format, we can change it as we go on to suit our needs.

Seaside Fire Department August 22, 2009 Responded to a structure fire, 1300 hours, single family dwelling. While in route dispatched informed responding units that a chair was on fire inside the residents. Light smoke coming from the front door, PD informed fire personnel that all the occupants were out of the house and a neighbor had put the fire out. Firefighter 1 grabbed a PW extinguisher and firefighter 2 prepared for horizontal ventilation. On the way to the front door firefighter 1 was stopped by the neighbor, and was told it would be easier to make entry through the rear of the structure, using the side yard gate. Captain and firefighter 1 found a fully involved recliner (chair) and a bin of news paper next to a wood burning stove with heavy smoke exiting the rear of the structure. Using the PW extinguisher the fire was controlled and contained to the chair and news paper bin. Second due established IRIC and other duties to include utilities, and treat one of the residents for smoke inhalation. The Captain would conduct a primary search while fire was being controlled. The chair and news paper bin was pulled outside for overhaul using the residents garden hose.
Is this what you are looking for. I'm not sure if grabbing the PW was the right chose, it worked. What would my big brothers have done. This was a fast deployment. Night time would have definitely changed the way we would have handled it, we might have found a fully involved house with two rescues.

Brian, Yeah thats what we're looking for. Normally I would say that a 1 can fire isn't something too post worthy unless there was a lesson learned or something, but since this is a new part of the forum why not!!
I would like to start a new thread for each fire so there can be discussion specific to the job. I will build a sample for all to see.
Again, this is just a trail basis for now. If anyone has anything to add, let me know via PM or here on the main thread...

Folsom State Prison Fire Dept. Oct 3, 2009 Stucture Fire Response- Heavy smoke in max security prison pod, craclking from behind a steel reiforced door in the plumbing chase between cells 134 and 135.
I responded with my engine company of 3 inmate FF and Rescue with 2 inmate FF. Arrived on scene, no smoke showing from exterior. The pod was evacuated of 70 inmates and 5 officers. Inside the pod we had thick white/ grey smoke with a crackling and roar from behind a steel rienforced door covering a pluming chase between cells 134/135. I knew we had electrical lines that were popping and they were live. My first course of action was to shutdown the electrical to no avail. My 2nd course was to see the extent of the fire and where it was going. Risking myself I had to open the door. I was in full structure PPE and so was my Personel, I had my men standing by with 2 10lb ABC Extingusihers and off to the side. I used the door as a shied as I made entry. I had in the back of my brain the recent LODD of the Santa Clara County Capt that got electricuted on a fire. I opened the door to find flames rolling out and energized lines swinging out toward us, I had my crew knock down the flames with the extinguisher. The lines never kicked off, it took 90min for an electrician to arrive and shut down the lines. I just had my crew keep the fire knocked down with ABC extinguishers to prevent extension. We cleared as soon as we ejected the smoke from the building. The fire was caused by faulty electrical wires that were secure to copper pipes. I know that we have a dangerous job, I know I put myself in harms way but I had to prevent that fire from effecting the other pods in the cell block. I thought that my gloves would insulate me enough from getting lit up through the door, I felt as long as I did not have direct contact with the wires I'd be okay. If I thought wrong let me know. I am a new officer and not an electrican. I'd appreciate any input. Thanks, STAY LOW AND LET IT BLOW!!! PTB-EGH

Folsom State Prison Fire Dept Last month Wildland Fire- Mutual Aid with Folsom City
Wildland Fire- about an acre or two, light flashy fuels and duff, moderate incline on the corner of prison road and notoma st. Responded in my grass rid, had 2 type ones. Batt Chief assigned my crew and I to take the left flank and attack the head of the fire which was threatning about five private residences. My guys did a good job, they were wearing the appropriate ppe and did a good job with the progressive hoselay. My veteran inmate FF parolled and the majority of my guys had about less than a month on the job. They flopped the first connection because they forgot to flake out the nas pak. We layed about 1100 ft of line. My engineers set up shuttle ops and supplied us and the city. During mop up we all 8 of my guys cutline around the whole perimiter as if they were a hand crew of 30 guys. My boys got in the grass and kicked some ass. Remember a few extra seconds of set up will save hours of dealing with mistakes. I let my guys know its not a race.

Sutter County Fire Station 5 area 12/19/09 0700 Hrs. Reported Structure Fire First Engine at scene reporting heavy smoke showing from a single story single family residence.
Two tank lines were pulled. Forced front door. Light smoke in the interior. Pulled ceiling and found active fire in the attack. Chased it in the attack space. As other crews arrived vertical ventilation and horizontal ventilation was preformed. Lots of overhaul due to celulose insulation. No one hurt. Everybody goes home.
